SUCCESSFUL ORTHOPEDIC SURGERY FOR A PEDIACTRIC PATIENT WITH AVASCULAR NECROSIS OF THE FEMORAL HEAD AND LEG FRACTURE DUE TO AN ACCIDENT

In 07/2023, 8-year-old L.H.M.T was brought to AIH by their parents for the situation of having shorter right leg compared to their left and limited movement.Upon clinical examination at the Orthopedic Trauma Department, Prof. Dr. René D. Esser diagnosed M.T. with Legg-Calvé-Perthes disease, also known as avascular necrosis of the femoral head in children. This condition causes misalignment in her right hip joint, leading to her right leg being approximately 3cm shorter than her left leg, which significantly impacted her mobility and daily activities.

Legg-Calvé-Perthes is a disease associated with disrupted blood flow to the femoral neck and head, causing necrosis. Over time, this condition may damage the joint structure, cause cartilage breakdown and deform the femur. It is commonly seen in children aged 4-10, with boys being 4 times more likely to develop this condition than girls.

According to the initial treatment plan, after preoperative tests returned normal, Prof. Dr. René D. Esser prescribed two surgical procedures for L.H.M.T. to correct the axis of the right femur and lengthen the right leg. The first surgery was successfully performed, and the patient was discharged after four days with stable vital signs, post-surgical pain was 3/10, and she was in the recovery phase before proceeding to the second surgery for limb lengthening. However, four months after the initial surgery, M.T. unfortunately experienced a fall, resulting in an angulated fracture in the right leg. The break occurred at the upper third of the femur, precisely at the site of the orthopedic fixation device implanted during the previous procedure.

Due to the new injury affecting the recovering femur, Dr. René and the team at American International Hospital (AIH) conducted a consultation and decided to remove the previous fixation and replace it with an Orthofix external fixator to realign the bone. A remarkable aspect of this surgery was Dr. René's use of the fracture site to perform limb lengthening for the pediatric patient. This technique required advanced expertise, exceptional precision, and significantly greater complexity compared to the initial treatment plan.

Notably, the Orthofix external fixator used in this procedure is a bone reconstruction method, and its applicatiobn on numerous complex cases was pioneered by Prof. Dr. René D. Esser. Over the past 30 years, Prof. Esser has taught this technique in Vietnam and globally, providing many trauma patients the opportunity for effective treatment and positive recovery. This method has several outstanding advantages, including: providing stable fixation for the injured part of the limb, allowing patients to move and start rehabilitation early while maintaining proper limb length; eliminating the need for biomaterials or synthetic bone grafts by utilizing the patient’s own bone for defect reconstruction; reducing the number of surgical procedures required, thereby minimizing the risk of infection and localized soft tissue damage.

The surgery for M.T. was successfully completed within just 2 hours. Over five months post-operation, X-ray images revealed good recovery, allowing the Orthofix frame to be removed. The patient's right leg was successfully lengthened, enabling the 8-year-old to walk normally on both legs without limping or having previous difficulty. This experience offers him brighter opportunities in life from now on.
