TWO SURGICAL TEAMS COLLABORATE ON SURGERY FOR A PATIENT WITH COMPLEX BILATERAL CLOSED FOREARM FRACTURES DUE TO A TRAFFIC ACCIDENT

The American International Hospital (AIH) has successfully received and managed a complex injury case: a concurrent complex closed fracture of both upper limbs due to a traffic accident. The patient, H.T.H (37 years old, residing in Thu Duc City), was transferred to the Emergency Department immediately after the accident, in a state of severe pain, swelling, and complete loss of motor function in both arms.

The X-ray and CT scan results show that the patient has complex injuries in both upper limbs, specifically a complex closed fracture of the lower end of the right radius and a complex closed fracture of the left olecranon. According to 1st-degree specialist, Dr. Nguyen Viet Thinh, these are two types of fractures that are very difficult to manage surgically in the upper limb, especially when joint involvement is present, this requires the surgeon to accurately restore the joint surface and anatomical morphology to prevent long-term complications such as joint stiffness, secondary osteoarthritis, reduced range of motion, and chronic pain.

Due to the severity of the injuries, two independent surgical teams were deployed in parallel to shorten the operation time and minimize complications related to prolonged anesthesia for the patient. Team 1, led by MSc. Dr. Nguyen Hong Trung - Medical Director, was primarily responsible for the right wrist area. And Team 2, led by 1st-degree specialist, Dr. Nguyen Viet Thinh - Deputy of General Surgery Department, was primarily responsible for the left elbow area.

Throughout the surgical procedure, the bone fracture sites are anatomically reduced and continuously monitored under the guidance of the image intensifier system (C-Arm), allowing the surgeon to directly and accurately observe the fracture location, the direction of plate and screw placement, as well as the stability of the bone structure. Thanks to this, the control of the fracture site and the osteosynthesis instruments achieves near-absolute precision, ensuring stable fixation results and optimal motor function recovery after surgery.

The surgery lasted 2 hours. Thanks to the seamless coordination and solid expertise of two surgical teams, along with comprehensive support from the modern equipment that meets international standards in operating room OR1, the entire process was tightly controlled and effective. This not only helped shorten the intervention time but also optimized the accuracy of surgical maneuvers, thereby enhancing treatment efficacy and minimizing the risk of postoperative complications. This is a key factor that contributes to helping patients recover quickly, safely, and comprehensively.

After surgery, the patient shared: “When I had the accident, I initially only felt pain in my arm, so I thought it was just a soft tissue injury. However, when I was transferred to the American International Hospital (AIH) and underwent X-rays and CT scans, I discovered that I had complex closed fractures in both arms, which made my family and me very worried. Fortunately, Dr. Trung and Dr. Thinh explained my injury condition and treatment plan very clearly, which helped me feel reassured and confident. Additionally, the surgery went very smoothly; I didn’t experience much pain, and the recovery process was quite fast. According to MSc. Dr. Nguyen Hong Trung - Medical Director, many cases of patients after traffic accidents show symptoms of dull pain, not much swelling, or can still move, leading to complacency and not seeking medical attention, resulting in missed closed fractures, especially in the upper limbs. However, in cases of closed fractures, especially intra-articular fractures or complex fractures, if not diagnosed and treated promptly, they can progress into serious complications such as bone displacement, pseudarthrosis, osteoarthritis, or loss of motor function… Therefore, patients after trauma, even if minor, also need to go to medical facilities with a Traumatology & Orthopedics specialty to be examined and undergo X-ray or CT scan in order to detect injuries early and have appropriate, timely treatment plans.

At the American International Hospital (AIH) Orthopedic Department, during the consultation and treatment process, doctors will apply the latest methods, techniques, and treatment regimens according to international standards.
