TIMELY EMERGENCY CARE FOR A GERMAN TOURIST WHO PRESENTED WITH COMA AND SEIZURES, WITH A HISTORY OF BRAIN SURGERY

12/10/2026

While traveling in Ho Chi Minh City, a 61-year-old German female tourist suddenly lost consciousness and developed coma and generalized seizures. Following timely emergency treatment at American International Hospital (AIH), the patient received intensive care. At the same time, AIH physicians proactively established contact with the hospital in Germany where the patient was receiving treatment, helping ensure continuity of care between Vietnam and Germany.

Patient in Deep Coma with Life-Threatening Condition

Upon admission to the AIH Emergency Department, patient W.A.K. (61 years old, German national) was in a deep coma with no motor response, with a Glasgow Coma Scale (GCS) score of E1V1M1 – 3/15. The patient also presented with stridor, a warning sign of impending airway compromise requiring urgent management.

Immediately upon admission, Dr. Strati Julian Alberto conducted a comprehensive assessment, provided oxygen support, and closely monitored the patient's vital signs, while prioritizing the maintenance of respiratory, circulatory, and neurological functions.

A medical history obtained from the patient's family revealed a history of hypertension and hypercholesterolemia. Notably, the patient had undergone brain tumor surgery approximately 2.5 months earlier in Germany due to a metastatic lesion associated with melanoma, a type of skin cancer arising from melanocytes. This information was important in helping physicians determine the possible cause and assess risks associated with the patient’s coma and seizures.

Comprehensive Assessment and Timely Management

Alongside emergency management, the physicians promptly ordered the necessary investigations, including a chest X-ray and brain CT scan, to assess the patient's overall condition, evaluate the central nervous system, and investigate potential causes of the coma and seizures in the context of her history of brain tumor surgery.

The patient was closely monitored and continuously managed according to the clinical progression. Following the interventions, the level of consciousness showed signs of improvement: the patient regained consciousness, was well oriented, and responded when addressed. The respiratory status also remained stable. At the same time, Dr. Strati Julian Alberto proactively communicated with the hospital in Germany where the patient was receiving treatment, supporting the provision of appropriate care at AIH.

Once the condition was initially stabilized, the patient was transferred to the Intensive Care Unit (ICU) for close monitoring and specialized care.

Cross-Border Continuity of Care

Thanks to intensive care in the ICU, the patient's health improved significantly, and the patient’s condition gradually stabilized. The patient was able to move independently with the support of the physical therapy team. After five days of hospitalization and monitoring, the patient was deemed fit for discharge and returned to Germany to continue the prescribed treatment plan.

Sharing the experience of the care provided at AIH, the patient's husband said:“We were very satisfied with the care provided by the medical team at AIH, especially Dr. Julian. During the emergency treatment and subsequent care, Dr. Julian demonstrated in-depth knowledge of neurological conditions and clearly explained my wife's condition to us. AIH also has modern medical equipment, including CT, MRI, and intensive care facilities. In particular, the hospital helped establish communication and exchange information with the hospital in Germany where she is receiving cancer treatment, which made coordination of her care much smoother. The physical therapy team provided excellent support with breathing exercises and walking exercises, which helped my wife recover quickly.”

The patient's husband expressed satisfaction with the treatment outcome at AIH, specifically praising Dr. Julian for his exceptional dedication and expertise.

For international patients receiving treatment for complex medical conditions abroad, emergency care involves more than simply managing the acute condition; it also requires effective information sharing and coordination between healthcare facilities. In this case, AIH proactively established communication and professional consultation with the hospital in Germany, providing the medical team with additional information about the patient's medical history and treatment course. This helped ensure continuity of care before and after the patient's return to Germany.

With 24/7 Emergency and Intensive Care services, AIH is ready to receive and promptly manage emergency cases while ensuring that patients have access to specialized care immediately after the critical phase.

At the AIH Intensive Care Unit (ICU), critically ill patients receive specialized intensive care in an environment equipped with modern medical technology, enabling continuous monitoring and timely management according to their clinical condition. The care process is standardized in alignment with international JCI standards, with patient safety at the center of every stage of treatment.

With a highly specialized medical team and multidisciplinary coordination capabilities, the AIH ICU is equipped to meet the care needs of critically ill and complex cases, as well as patients at risk of rapid deterioration.
