SUCCESSFUL EMERGENCY TREATMENT OF A PATIENT WITH LIVER ABSCESS AND SEVERE GASTROINTESTINAL BLEEDING, SAVED FROM A CRITICAL CONDITION

Recently, American International Hospital (AIH) successfully provided emergency treatment to Ms. Nguyễn Thị Mai Hương (74 years old, Bến Thành), who was admitted in critical condition with severe fatigue, shortness of breath, high fever accompanied by chills, right upper quadrant pain, and pale skin and mucosa. The emergency team immediately acted: the patient was given oxygen therapy, two intravenous lines were established, fluid resuscitation was initiated, and urgent laboratory tests and diagnostic imaging were ordered.

Clinical examination and paraclinical results revealed a large right liver abscess, accompanied by right-sided pleural effusion occupying two-thirds of the lung, causing severe dyspnea. The patient was also found to have coagulopathy and severe anemia suspected to be due to gastrointestinal bleeding from the stomach and duodenum. She reported having black stools for the past 2–3 weeks. According to Dr. Nguyễn Quang Duy, MD (Level I Specialist), Department of Emergency and Intensive Care, this was a severe case with a high risk of septic shock, abscess rupture, and life-threatening complications if not treated promptly.

A multidisciplinary consultation was immediately activated, involving Emergency & Intensive Care, General Surgery, Medical Gastroenterology and Nutrition, to determine the optimal treatment plan. The top priorities were stabilizing vital signs and managing severe dyspnea due to pleural effusion, as well as correcting coagulopathy and severe anemia caused by gastrointestinal bleeding. The patient underwent pleural drainage, blood transfusion, correction of coagulation disorders, and gastroscopy, revealing a large duodenal ulcer, and endoscopic hemostasis was performed in combination with medical therapy. Subsequently, Dr. Võ Bích Đại Hào, MD (Level II Specialist), Head of the Department of General Surgery, treated the liver abscess using a minimally invasive approach - ultrasound-guided percutaneous abscess drainage - which rapidly reduced the infection and minimized the risk of complications.

MINIMALLY INVASIVE APPROACH – ULTRASOUND-GUIDED ABSCESS DRAINAGE PROVIDES HIGH EFFICACY AND SAFETY FOR PATIENTS 

Dr. Võ Bích Đại Hào, MD (Level II Specialist), performed ultrasound-guided percutaneous drainage of the liver abscess under local anesthesia only, using a very small puncture site (3–4 mm). A total of 175 mL of purulent fluid was aspirated. The pus sample was sent for culture and antibiotic susceptibility testing to enable precise antibiotic selection. The abscess cavity completely collapsed; the patient became afebrile, experienced rapid pain relief, and, notably, did not require surgery under general anesthesia, which carries significant potential risks.

By avoiding general anesthesia and minimizing tissue damage, this technique reduces pain, leaves no large scar, shortens hospital stay, and supports faster recovery. This represents an advanced trend in liver abscess management and is routinely implemented at American International Hospital (AIH), delivering high treatment effectiveness for patients.    

In parallel with medical treatment, the patient, who had adhered to a strict vegetarian diet for over 20 years, was severely malnourished. Dr. Lê Phạm Anh Vy, MD (Level I Specialist), Head of the Department of Nutrition, developed an individualized nutrition plan with daily monitoring and adjustments. Upon discharge, the patient was further counseled on a transitional nutrition plan to support sustainable physical recovery and long-term health improvement.

After just one week of intensive treatment and close multidisciplinary coordination, the patient’s condition improved markedly, laboratory parameters stabilized, her health recovered rapidly, and she was discharged from the hospital.

According to Dr. Nguyễn Quang Duy, MD (Level I Specialist), Department of Emergency and Intensive Care, if not detected and treated promptly, a liver abscess may rupture, leading to widespread infection and potentially life-threatening complications. Therefore, when experiencing warning signs such as prolonged fever, right upper quadrant pain, fatigue, or loss of appetite, patients should seek medical attention promptly to avoid dangerous complications.
