A seemingly simple slip or fall in daily life can become a major event for the elderly, threatening not only their mobility but even their life. Recently, at American International Hospital (AIH), the Orthopedic team successfully performed a cementless hemiarthroplasty for a 90-year-old woman with a complex femoral neck fracture and multiple comorbidities, including triple coronary artery stents. The surgery helped the patient recover quickly and regain her quality of life.

A dangerous situation: femoral neck fracture at age 90

Patient L.T.N (90 years old) showed up with severe pain and was unable to move after slipping and falling. Master, Doctor Nguyễn Hồng Trung – Chief Medical Officer of AIH, from the Orthopedic Department, directly examined the patient and ordered the necessary diagnostic tests. The results showed a complex right femoral neck fracture, grade IV.

At the age of 90, with frail health, if not treated promptly, the patient would be at high risk of prolonged immobility, leading to serious complications such as pneumonia, pressure ulcers, malnutrition, etc.

Multidisciplinary Consultation and Indication for Hip Replacement Surgery

According to Master, Doctor Nguyễn Hồng Trung, for elderly patients, bone fixation methods using nails or plates often yield limited results. In this case, hip replacement was the optimal choice to help the patient regain mobility early and prevent complications from prolonged bed rest. Notably, the patient also had multiple underlying conditions, including ischemic heart disease with coronary stent placement, ongoing anticoagulant therapy, chronic obstructive pulmonary disease, diabetes, etc., which made anesthesia and surgery more complex.

Master, Doctor Nguyễn Hồng Trung decided to perform a cementless hemiarthroplasty – a type of prosthesis designed to be anchored directly into the bone marrow canal, allowing natural bone growth to adhere firmly to the implant surface through a biological fixation mechanism. This method helps reduce cement-related risks in elderly patients with cardiovascular disease while providing durable stability for the hip joint.

Before surgery, the patient discontinued anticoagulant medication for five days, received pain management, continued treatment for comorbidities, and was given psychological support. At AIH, treatment for such special cases is carried out under a multidisciplinary care model, minimizing risks for elderly patients with multiple health conditions.

The surgery was performed by Master, Doctor Nguyễn Hồng Trung and the Orthopedic Surgery team. Within 24 hours after surgery, the patient was able to resume anticoagulant therapy and begin physical therapy. After three days, she recovered rapidly and started walking with crutches.

When Is Hip Replacement Surgery Necessary?

According to Master, Doctor Nguyễn Hồng Trung, hip replacement surgery (partial or total) is an effective treatment for severe hip joint damage caused by trauma or pathological conditions of the femoral neck that result in significant pain and restricted movement. Common indications include:

  • Femoral neck fracture in the elderly: brittle, weak bones make fixation with screws or nails difficult to heal, and prolonged immobilization can lead to pneumonia, pressure ulcers, malnutrition, urinary tract infections, etc.

  • Progressive hip osteoarthritis: caused by aging, chronic arthritis, ankylosing spondylitis, or as a complication of tuberculous arthritis.

  • Avascular necrosis of the femoral head: loss of supply causes the femoral head to collapse and the hip joint to deform, leading to severe pain.

  • Congenital or acquired hip deformities: significantly impair mobility and posture.

  • Complications from chronic diseases such as gout, diabetes, or sequelae from previous hip injuries.

Hip replacement surgery offers multiple benefits: it significantly relieves pain, restores mobility, improves quality of life, and most importantly, prevents dangerous complications associated with prolonged immobility.

Hip osteoarthritis can occur in anyone, but certain factors significantly increase the risk:

  • Elderly individuals.

  • People with a history of hip injury (such as femoral neck fracture, dislocation, or acetabular fracture).    

  • Those with inflammatory joint diseases (hip arthritis, ankylosing spondylitis, or tuberculous arthritis).
