SAVING A PATIENT WITH RECURRENT MENINGIOMA, PROMPTLY PREVENTING HEMIPLEGIA

At American International Hospital (AIH), Dr. Nguyen Manh Hung, Specialist Level I – Head of the Department of Neurosurgery and Spine, performed a successful surgery on a patient with a third recurrence of a right-hemisphere meningioma, promptly preventing hemiplegia and preserving the patient’s mobility. 

Patient Tram Bich Van (68 years old, Tay Ninh) underwent two brain surgeries over two years. Initially, the patient only presented with hand spasm, then felt hand weakness gradually. Primary diagnostic imaging detected a meningioma in the right parietal region, directly related to the motion region. The first surgery completely removed the tumor, with the pathological report confirming tumor grade 3. After six months of monitoring, recurrent symptoms, including headache, hand spasm, and hand weakness, appeared more severe compared to the first time. The second surgery was performed promptly, managing the complications. 

Before the third surgery, within two months, the patient consecutively re-experienced severe symptoms like prolonged headache, reduced sensation, weakening, and left-bodily convulsion. 

The patient rushed to the examination at AIH with Dr. Nguyen Manh Hung, Specialist Level I – Head of the Department of Neurosurgery and Spine. Based on clinical symptoms and MRI results, Dr. Hung diagnosed the patient with recurrent right-hemisphere meningioma (grade 3), affecting perception, mobility, and sensation.

Managing complicated brain tumor and preserving mobility and sensation

With a history of two brain surgeries, Dr. Nguyen Manh Hung, Specialist Level I, stated: Regarding the third recurrence of meningioma, the patient presented with signs of weakness and hemiplegia, muscle flaccidity, indicating severe progression. If surgery is postponed, there are high risks of severe episodes of epilepsy, coma, or even death.

Immediately, Dr. Nguyen Manh Hung, Specialist Level I, created a comprehensive treatment plan, including craniotomy combined with advanced microsurgery techniques.  

The recurrent tumor had a large size (42×36×41 mm), adherent to the healthy brain parenchyma, and was dissected under the operating microscope, enabling complete removal of the tumor, maximal preservation of healthy tissue, and reduced risk of complications. Simultaneously, the surgeon reconstructed the dura and skull using titanium mesh, helping protect the brain, stabilizing the skull shape, and ensuring appearance for the patient.

The strategy for this surgery focused on 2 objectives: first, to remove the tumor completely under the microscope. This is a complicated tumor with multiple adhesive scar tissues from previous surgeries, requiring an extremely meticulous technique to avoid damaging benign tissue. Second, optimally preserving and repairing mobility, sensation, which help the patient to reintegrate into daily life” - Dr. Hung shared.

When awakening, the patient felt fully alert and could follow the doctor’s order, slightly move the limbs. After three days of hospitalization, the patient showed improvement in movements and communication.

On the third day post-op, the patient noted a quick recovery, presenting comfort in communicating with their loved ones. 

Following up after 10 days, the patient illustrated nearly normal ambulation, clear communication, and significantly improved consciousness. Especially, thanks to the skillful technique of Dr. Nguyen Manh Hung, Specialist Level I, the patient’s hair was kept intact, helping the patient look as if she had never undergone any life-threatening operation.
