PROMPT EMERGENCY TREATMENT FOR THE KOREAN PATIENT WITH NECROTIC RETROCECAL APPENDICITIS
Appendicitis is an acute disease that requiring attention to its symptoms and manifestations for early diagnosis and timely surgical intervention in order to avoid complications and mortality risk.
Recently, American International Hospital (AIH) has received a case of necrotic retrocecal appendicitis (the proximal end of colon connects to the distal end of small intestine) in the Korean patient named Kim - 24 years old, Korean nationality. According to the patient's representative in Vietnam, while working on a ship from Thailand to Vietnam, Mr. Kim had increasing abdominal pain within 5 days (severe pain started from day 3). The pain initially occurs in periumbilical region, then radiates to the right iliac fossa. Be aware of severe pain may influence on his work, Kim's colleagues quickly let the ship dock at Cat Lai port and contacted the American International Hospital (AIH) to arrange an ambulance for prompt patient pick-up.
Mr. kim had been admitted to the Emergency Department for severe abdominal pain, medical examination was given by AIH’s general surgeons with the signs of right iliac fossa pan, abdominal guarding noticed, possible complications of localized peritonitis, abdominal CT-scan immediately indicated. CT-scan result revealed necrotic appendicitis, fecaliths impacted and local pelvic peritonitis (infracecal and ascending to retrocecal position). At this time, the patient is counseled and arranged for emergency surgery in 02 hours only.
During surgery process, it was found that the retrocecal appendix tightly attached to the lateral peritoneum, swollen appendix with many necrotic positions which can cause dangerous complications if treatment is not promptly given such as extended peritonitis becomes generalized peritonitis, a serious infection.
At the American International Hospital (AIH), most of appendectomy use laparoscopic method with the advantages of less pain, small incision scars and quick recovery time.
