Meningococcal disease is a dangerous acute bacterial infection caused by the bacterium Neisseria meningitidis. The disease is characterized by a rapid progression within just a few hours, leading to clinical manifestations of purulent meningitis and septicemia with a high risk of mortality.

What is Meningococcal disease?

Neisseria meningitidis (the meningococcus) is a bacterium that colonizes the human nasopharynx and is transmitted via respiratory droplets during close contact, such as coughing, sneezing, sharing food or utensils, or speaking at close range.

The disease presents with a sudden onset of severe symptoms, including high fever, intense headache, nausea, vomiting, and stiff neck. Particularly, the appearance of rapidly spreading, stellate purpura, sometimes with bullous lesions. In fulminant cases, the disease may manifest as lethargy, coma, septic shock, and multi-organ failure, leading to rapid death within 24 hours.

Early warning signs

  • Incubation period: Typically 3–4 days (range: 1–10 days)

  • Initial symptoms: Early manifestations may mimic common influenza, leading to underestimation of severity. However, particular attention should be paid to the following signs:

    • In adults and older children: Sudden onset of high-grade fever, severe headache, stiff neck, difficulty in neck flexion, nausea, vomiting, sensitivity to light, confusion, drowsiness.

    • In young children: Poor feeding, irritability, bulging fontanelle (in neonates), seizures, lethargy, or abnormal agitation.

    • Purpura: Red or dark purple rash, non-blanching, irregular borders, potentially with central necrosis; sometimes the purpuric lesions may present as blisters, which aids in diagnosis.

    • Fulminant cases: Severe infection, rapid progression to shock, early agitation or coma, with purpura appearing early and spreading rapidly.

How does the disease spread?

Meningococcal disease can occur year-round, but it often increases during seasonal transitions, especially in communal environments such as daycare centers, schools, and dormitories. Meningococcal bacteria are primarily transmitted through the respiratory route, via close contact with carriers when they cough, sneeze, or talk. Carriers may be asymptomatic yet still serve as a source of infection for the community. Droplets containing the bacteria spread through the air and enter the body through the nose or throat of those in close contact. Additionally, the disease can be transmitted indirectly through objects contaminated with respiratory secretions from a carrier, particularly when the living environment is not kept clean.

Effective prevention methods

Since the disease is transmitted via droplets, it is essential to implement the following preventive measures:

  1. Maintain hygiene and ensure proper ventilation in living and working spaces.

  2. Practice good personal hygiene: wash hands frequently with soap and water; refrain from spitting in public; and cover your nose and mouth when coughing or sneezing.

  3. Wear a face mask when ill, when in close contact with an infected person, or when visiting crowded places, especially hospitals.

  4. If you have had close contact with a patient suffering from meningococcal disease, notify local health authorities to receive prophylactic antibiotics within 24 hours.

  5. Vaccination provides active immunity, reducing the risk of infection and severe complications. Vaccination is recommended for both children and adults and is currently considered the most effective preventive measure.

Although meningococcal disease is dangerous, it is entirely preventable if we proactively get vaccinated and maintain good hygiene habits. Early recognition of abnormal signs and seeking timely medical attention will help minimize risks.
