The challenges in treating patients with spinal fracture and multiple underlying conditions

Before arriving at AIH, patient N.T.P (55 years old, Dong Nai) fell and experienced severe back pain and weakness in both legs. The patient had been receiving medical treatment at a provincial hospital for a month with diagnoses of pneumonia, spinal fracture, osteoporosis, diabetes, Cushing's syndrome, atrial fibrillation, hyperthyroidism, gout, and general asthenia. However, after a period of treatment, the pain did not subside but increased, and the patient was almost unable to sit or walk.

Recognizing the unusual progression, the patient and family contacted Dr. Nguyen Manh Hung, 1st-degree specialist, Head of the Neurosurgery and Spine Unit at American International Hospital (AIH), for consultation. Through discussion, Dr. Hung determined that this could be a complex spinal fracture causing severe complications, and advised transferring the patient to AIH immediately same-day for emergency treatment.

30-minute “lightning-fast” surgery

At AIH, after clinical examination and based on diagnostic imaging results, Dr. Nguyen Manh Hung diagnosed the patient with multi-level spinal fractures, compression fractures of the D5, D6, L1, and L4 vertebral bodies, and spinal cord - cauda equina compression, this is a severe neurosurgical emergency with a risk of permanent paralysis if not intervened upon promptly. Previously, these injuries were not correctly diagnosed and treated, leading to increasing pain, limited movement, and a rapidly progressing risk of neurological damage.

After surgery, the patient was monitored and actively managed for underlying conditions. After only a few days, the patient was able to sit up independently, perform light exercises, and quickly recovered, being discharged in stable condition. At the follow-up appointment two weeks later, the patient reported having returned to normal activities and was very satisfied with the treatment outcome.

The patient is at risk of spinal instability, progressive spinal cord damage, lower limb paralysis, sphincter disorder, along with a series of secondary complications due to prolonged immobility such as pneumonia, pressure ulcers, or deep venous thrombosis.

The technique of balloon kyphoplasty and biological cement injection at AIH

Balloon Kyphoplasty is a superior advancement in the treatment of vertebral compression fractures – a serious complication often seen in patients with osteoporosis or spinal trauma. During the surgery, the doctor uses a specialized balloon inserted into the vertebral body through a small needle, inflates the balloon to create a cavity and restore the collapsed part of the vertebral body to its original shape. Subsequently, biological cement is injected into this cavity under controlled pressure, helping to stabilize the bone structure, restore vertebral body height, provide immediate pain relief, and prevent further progression of spinal cord - nerve root compression.
