HIP REPLACEMENT FOR AN 88-YEAR-OLD CAMBODIAN PATIENT WITH FEMORAL NECK FRACTURE

Recently, American International Hospital (AIH) performed a partial hip replacement for patient S.T. (an 88-year-old Cambodian) who previously experienced a fall resulting in a right femoral neck fracture. The patient was transferred to AIH with a fractured femoral neck, a coronary stent, significant pain and multiple other underlying conditions: pneumonia, renal and adrenal failure, prostatic enlargement, anticoagulant use, Cushing syndrome due to prolonged corticosteroid use… The condition was specified as severe due to patient’s advanced age and numerous underlying conditions. AIH arranged a hospital-wide consultation among specialists from various departments such as Orthopedics, Anesthesiology, ICU, Internal Medicine and Nutrition to provide a comprehensive evaluation, screening, and personalized treatment plan for the patient.



A sharing from AIH’s orthopedic specialist: “Femoral neck fractures in older adults are severe types of injuries and in need of timely intervention and treatment to avoid bedridden complications like deep vein thrombosis (DVT) in the lower leg, pressure ulcers, infections (pneumonia, urinary tract infection), amyotrophy, and malnutrition. Partial hip replacement is an effective treatment in this case due to short operative time and quick pain recovery which help patients with early mobilization and reduce the risk of such complications. However, performing surgeries on older patients with multiple and severe underlying conditions presents great challenges that require not only advanced expertise from the surgeon but also safety in anesthesia during the procedure and patient’s recovery and mobility function postoperatively. At the same time, operation hours must be minimized to avoid risks of complications associated with anesthesia and blood loss during the surgery. 

After thorough consultation, where the doctors explained the benefits and risks of the surgery, the patient and his family consented to the treatment. The surgeon team successfully performed a partial hip replacement for the patient within 30 minutes. He was then transferred to ICU for further monitoring and postoperative care. After 10 days of intensive care and physical therapy in the hospital, his condition temporarily stabilized. He experienced no pain, had good leg movement, and a dry surgical wound so he was discharged at his family’s request.



Femoral neck fractures in older adults are more prevalent due to osteoporosis. Because of low bone density, bones are brittler and prone to fractures even with minor trauma or improper movements. This is a severe type of injury, which is challenging to heal and usually associates with multiple complications. Therefore, according to orthopedic specialists, if an older person in the family suffered an injury following a fall, the person should be promptly taken to the nearest medical facility capable of diagnosing and treating. This helps prevent risks of disability and reduced life expectancy in the elderly. Additionally, unsupervised treatment methods are not recommended. Moreover, families can routinely monitor elderly patients’ health and arrange frequent general health checkups to detect and prevent bone and joint diseases, ensuring a better quality of life for the elderly.
