BE CAUTIOUS OF FOOD ANAPHYLAXIS IN CHILDREN

On April 23, 2025, American International Hospital (AIH) received a case of a 5-year-old boy with signs of red rash, discomfort, abdominal pain, and diarrhea. Through history taking, it was learned that the boy was previously healthy; however, about 30 minutes after eating frog porridge at school, scattered red rash appeared all over his body, along with wheezing, cough, discomfort, abdominal pain, and diarrhea once. Symptoms progressed quickly, and the rash spread all over his body, causing intense itching. The boy did not have a fever. Immediately after detecting the abnormality, the teacher applied medicated oil to the boy and contacted the parents. Realizing the symptoms were not like common allergies, the mother immediately took the boy to AIH, instead of self-treating as in some other cases.

At the pediatric outpatient department, doctors diagnosed the boy with suspected foodborne anaphylaxis grade 2. The boy was given emergency treatment according to the regimen with: intramuscular adrenaline injection, antihistamines (Dimedrol), and Methylprednisolone. After management, the boy's condition clearly improved: wheezing stopped, rash decreased, and abdominal pain ceased. The boy was hospitalized for observation due to the risk of a biphasic anaphylaxis. After 2 days of treatment, the boy was healthy, eating normally, and was discharged with a scheduled follow-up for subsequent allergy testing.

  • Severe allergic reactions can occur even with foods that the child has eaten before without any issues.

  • Do not be subjective and self-treat at home with anti-allergic medications, especially for young children - because the golden hour for anaphylaxis treatment might be missed.

  • Some parents may have seen relatives develop rashes after eating seafood and think it's "not serious," but anaphylaxis can progress quickly and be life-threatening if not treated promptly.

 Signs of suspected anaphylactic shock:

After eating (usually within 30–60 minutes), a child may develop:

  • Rapidly spreading itchy rash

  • Wheezing, difficulty breathing

  • Abdominal pain, vomiting, diarrhea

  • Lethargy, fatigue, dizziness.

 Role of school and teachers:

  • Early detection of unusual symptoms is a very important factor that helps children receive timely emergency care.

  • Teachers, school health staff, and parents need to be equipped with basic knowledge to recognize signs of anaphylactic shock, as many severe reactions can occur at any time and in any place.
