UTERINE PRESERVATION FOR A SECOND-TIME PREGNANT PATIENT WITH PLACENTA ACCRETA
High-risk pregnancy managed closely
Pregnant patient L.H.U. (41 years old, HCMC), pregnant for the second time, has a history of C-section. Simultaneously, the Obstetrics-Pediatrics team at AIH proactively planned a C-section at 37 weeks, preparing sufficient manpower, anesthesiological resources, and blood supplies to ensure maximum safety.
Activating the "red alert," managing with "lightning speed"
One day before the scheduled surgery, the pregnant patient experienced sudden, massive hemorrhage and was rushed to AIH for emergency care. Upon admission, the hospital activated the "Red Alert" protocol, urgently mobilizing the Obstetrics, Anesthesiology teams, and blood bank.
The surgery was meticulously prepared with backup plans, including the mobilization of blood supplies and blood products for immediate replacement. The surgical and anesthesiology teams were on standby, coordinating closely to manage the risk of severe hemorrhage. Thanks to a rapid response and seamless coordination during the "golden hour", the surgery was performed in a timely manner.
Dr. Vu Van Phi, Specialist Level I, successfully managed the case of placenta previa accompanied by placenta accreta, effectively controlling the bleeding, transfusing 02 units of blood, and importantly, preserving the patient's uterus. The baby boy was born healthy, weighing 3.1 kg, bringing overwhelming joy to both the family and the medical team.
Dr. Phi stated: "If not intervened in a timely manner, the patient could face the risk of severe postpartum hemorrhage due to the condition of placenta previa combined with placenta accreta. In many cases, to control the bleeding, doctors are forced to perform a hysterectomy, resulting in the loss of future fertility."

Dr. Phi stated that placenta accreta is a dangerous complication with an increasing incidence (approximately 1/500 to 1/1,000 pregnancies), commonly occurring in patients with a history of C-sections or uterine interventions. This condition causes the placenta to adhere deeply, making it difficult to detach after delivery and prone to causing severe postpartum hemorrhage if not monitored and managed promptly.
Therefore, high-risk pregnant women need regular prenatal check-ups and should choose a medical facility with strong emergency response capabilities. When experiencing abnormal signs such as vaginal bleeding or abdominal pain, it is necessary to go to the hospital immediately to ensure the safety of both mother and baby.
