SUCCESSFUL HIP ARTHROPLASTY IN AN ELDERLY PATIENT WITH MULTIPLE UNDERLYING MEDICAL CONDITIONS

Femoral neck fractures in the elderly are a serious injury, especially when accompanied by multiple underlying medical conditions. Recently, Dr. Nguyen Hong Trung, Master’s Degree - Specialist in Orthopedic Trauma, Medical Director at the American International Hospital (AIH) - successfully performed a partial hip arthroplasty for an elderly patient, helping the patient overcome a critical period and restore mobility.

Treatment challenges in high-risk patients

Patient Le Thi Hoa (90 years old, HCMC) was hospitalized following a domestic accident and diagnosed with a fracture of the right femoral neck. Upon examination and evaluation, Dr. Nguyen Hong Trung indicated a hip arthroplasty to help the patient recover her mobility early.

However, this was a complex clinical case involving multiple risk factors, such as advanced age, numerous underlying medical conditions, coronary insufficiency, ischemic heart disease, chronic pneumonia, and chest CT scan results showing suspicious signs of a pulmonary embolism and the patient was on anticoagulant therapy. These factors significantly increased the risk of bleeding, as well as cardiovascular and pulmonary complications during anesthesia, intraoperative, and postoperative period. Therefore, after a thorough risk-benefit assessment, the surgical team unanimously decided to perform a partial hip arthroplasty for the patient.

To ensure absolute safety for the surgery, Dr. Nguyen Hong Trung organized and chaired a hospital-wide consultation with the participation of specialists in Orthopedics, Intensive Care, Cardiology, Anesthesiology, Nutrition,… to comprehensively assess the patient's condition and formulate the most suitable treatment plan. 

After careful consideration, the surgical team unanimously agreed to perform a partial hip arthroplasty to help the patient restore mobility and minimize complications.

Without surgery, the risk of complications is even greater

According to Dr. Nguyen Hong Trung, in elderly patients with femoral neck fractures, conservative treatment usually does not yield optimal outcomes.

Without surgery, patients will experience prolonged pain, loss of mobility, and complete dependence on caregivers. Long-term immobilization also increases the risk of pressure ulcers, pneumonia, urinary tract infections, debility, deep vein thrombosis, and embolism. These are life-threatening complications, especially in elderly patients with multiple underlying medical conditions.

Successful surgery thanks to multidisciplinary coordination

After the underlying medical conditions were controlled, Dr. Nguyen Hong Trung performed a partial hip arthroplasty. The operation was successful and lasted for 60 minutes. 

Opting for a partial hip arthroplasty helps shorten the surgical time, limit blood loss, and reduce the risk of complications in elderly patients. Throughout the treatment process, the patient was closely monitored by the Anesthesiology team, the Cardiovascular Intensive Care team, as well as other relevant specialties to ensure safety.

Following the surgery, the patient received pain management, surgical wound care, proper nutrition, and early physical therapy. After only 4 days of treatment, the patient's health stabilized, and met the discharge criteria.

At the 2-week follow-up examination, the patient's condition was evaluated, showing good mobility; the patient was able to walk with crutches, surgical site pain was negligible, and X-ray results confirmed that the hip joint was in a stable position. The patient continued the physical therapy program as instructed by the doctor.

According to Dr. Nguyen Hong Trung, Master’s Degree, the successful treatment of femoral neck fractures in the elderly depends not only on surgical techniques but also requires a comprehensive evaluation, good control of underlying medical conditions, and multidisciplinary coordination.
