“A HIGH-PRESSURE MANAGEMENT CHALLENGE”: CARING FOR A PREGNANT PATIENT WITH PRETERM PREMATURE RUPTURE OF MEMBRANES AT 32 WEEKS
Recently, the Department of Obstetrics and Gynecology at American International Hospital (AIH) faced nearly three weeks of intensive monitoring and maintenance of pregnancy for a pregnant patient who experienced preterm premature rupture of membranes at 32 weeks of gestation. Each additional day of pregnancy provided another opportunity for the fetus to further develop vital organs and helped reduce the complications associated with prematurity.
One More Day, One More Hope
Patient N.T.K. (35 years old, HCMC) was pregnant with her second child and had a history of C-section. Throughout her pregnancy, she received prenatal care at American International Hospital (AIH). At 32 weeks of gestation, she unexpectedly experienced preterm premature rupture of membranes and was admitted to the hospital for monitoring.
As soon as the patient was admitted, Dr. Vu Van Phi, Specialist Level I, Deputy Head of the Obstetrics and Gynecology Department, along with his medical team, promptly activated the high-risk pregnancy management protocol. They conducted a comprehensive evaluation of the gestational age, the conditions of both the mother and the fetus, the risks of infection and preterm labor, as well as the feasibility of safely prolonging the pregnancy. On that basis, a treatment and complication-prevention plan was implemented from the very first hours of hospital admission with the objective of maintaining the pregnancy within safe limits, buying every precious day for the fetus to continue developing in the uterus.
During the maintenance of pregnancy, the pregnant patient was monitored for signs of infection and preterm labor. The doctors regularly checked the mother’s temperature, blood pressure, and heart rate; performed blood tests to assess for inflammation and infection; and monitored uterine contractions and amniotic fluid volume. The fetus was continuously monitored through ultrasounds and fetal heart rate monitoring, and evaluation of intrauterine adaptation.
Alongside efforts to prolong the pregnancy, the Department of Pediatrics and Neonatal Intensive Care Unit (NICU) team remained on fully prepared. In cases of preterm premature rupture of membranes at 32 weeks of gestation, preterm birth can occur at any time. Through proactive measures across multiple dimensions, both mother and baby are consistently safeguarded under the safest possible conditions at AIH.
Thanks to close monitoring and the patient’s perseverance, the pregnancy was successfully maintained for nearly 3 precious additional weeks, from 32 to 35 weeks of gestation. The baby was born healthy and remained stable after birth, bringing great joy to the family and the medical team.
