02 SURGERIES TO REMOVE LARGE UTERINE FIBROIDS
Recently, Mmed. 2nd-degree specialist. Dr. Ho Nguyen Tien, Head of the Obstetrics and Gynecology Department, American International Hospital (AIH), successfully performed 02 laparoscopic surgeries to remove large and multiple uterine fibroids by in-bag morcellation, the condition caused complications such as menorrhagia and anemia for the patients.
In the first case, patient T.N.M (40 years old, Ho Chi Minh City) has a history of having undergone surgery for endometriosis. During her menstrual cycle, she often experiences severe abdominal pain. Recently, the patient showed signs of prolonged menorrhagia and therefore went to the hospital for examination. At AIH, the patient underwent physical examination, paraclinical tests. The results from ultrasound and CT scans indicated that the patient had a endometrial polyp, multiple large fibroids (10 cm), and an endometrioma on the left ovary. Dr. Ho Nguyen Tien consulted with specialists and recommended that the patient be hospitalized for surgical treatment of the tumor. The doctor performed two surgical techniques: laparoscopic ovarian polypectomy and combined with laparoscopic myomectomy, endometriotic cystectomy while preserving the uterus for the patient. According to Dr. Ho Nguyen Tien, the surgery was quite complex due to adhesions and previous surgical history, as well as multiple fibroids (10 fibroids). Two days after surgery, the patient’s health improved significantly, and she was discharged according to the doctor’s instructions.
In the second case, patient H.C.T (45 years old, a Vietnamese British) showed signs of prolonged menorrhagia and frequent fatigue due to anemia. Previously, the patient had undergone examinations in the UK but later chose to return to Vietnam for treatment with the desire for laparoscopic surgery to treat uterine fibroids. At AIH, ultrasound and CT results indicated that the patient had a large uterine fibroid measuring 10 cm and severe anemia caused by the fibroid, the doctor recommended an early laparoscopic myomectomy to avoid dangerous complications. After 02 days of hospitalization, the patient was stable to be discharged, experiencing minimal pain.
► Laparoscopic myomectomy
Both cases of laparoscopic myomectomy, Dr. Ho Nguyen Tien applied the method of in-bag morcellation that ensures tumor cells do not spill into the abdominal cavity. Morcellation is a surgical technique commonly used in laparoscopic surgery, particularly effective in cases such as laparoscopic partial hysterectomy, or resection of large tumors,…. In this technique, a large mass (usually a tumor or pathological tissue) is dissected and collected into a specialized bag, then it is morcellated into smaller pieces and removed from within a protective bag. The advantage of this technique helps reduce the risk of malignant cells spilling into the abdominal cavity, diminishes the time required for abdominal irrigation, and minimizes damage to organs within the abdominal cavity while removing the tumor.
Previously, the technique of morcellation without protective bag; in the case of malignant tumors, tumor cells were scattered throughout the abdominal cavity, leading to distant metastasis of malignant cells.
In addition, at AIH, there is an internationally standard OR1 operating room equipped with a modern Telemedicine endoscopic system featuring the latest generation multi-screen technology. This advanced imaging technology provides clear and detailed images, helping doctors to observe better during surgery. With rapid data transmission technology, doctors can conduct live reports and remote consultations quickly, allowing them to intervene and guide the surgical team in handling complications promptly and intuitively, easily.
► Effective Prevention of Uterine Fibroids
According to Dr. Ho Nguyen Tien, uterine fibroids, also known as uterine leiomyomas, are a benign condition of the uterus and are commonly found in women of reproductive age between 30 and 50 years old. When uterine fibroids are present, we need to pay attention to their location, size, and number. A person can have one or more fibroids at various locations and sizes simultaneously. A fibroid can remain very small for a long time before growing rapidly or developing slowly over many years. The symptoms of the condition also depend on the location and size of the fibroid; patients may experience symptoms such as menstrual disorders, pressure from the tumor on nearby organs, lower abdominal pain, and infertility… If uterine fibroids are not detected in time, treatment may be difficult and may lead to dangerous complications such as prolonged hypermenorrhea, menorrhagia, metrorrhagia that results in acute or chronic anemia, causing severe anemia and reducing the quality of life for the patient…
Therefore, doctors recommend that women should regularly undergo health check-ups, gynecological examinations every 6 months to 1 year, and undergo screening tests to detect gynecological diseases early. At the same time, when patients experience symptoms such as an abnormal enlarged abdomen, abdominal pain, uterine bleeding, menorrhagia, metrorrhagia, pelvic compression,… they should visit a medical facility for examination as soon as possible. If uterine fibroids are detected, patients need to be monitored and treated when complications arise. In cases where the tumor is large and surgery is required, depending on the patient’s age and the size of the fibroid, the doctor will decide whether to perform a hysterectomy or myomectomy to preserve the uterus for the patient.
At American International Hospital (AIH), a team of experienced obstetricians with high expertise will directly examine and diagnose, effectively treat gynecological diseases using modern equipment; most gynecological surgeries are performed laparoscopically. Many cases of large uterine fibroids with complex complications have been successfully operated on, improving the quality of life for women.
